Its different every where you go.
Some states do not allow any minor to be tattooed or pierced, no exceptions.
Other states allow you to get piercings and tattoos with parental/legal guardian consent, but they have to be present with you at the time of the procedure. Its not enough to just sign a piece of paper and bring it in. As far as the minimum age for that, its variable based on the state. Some will allow as low as 12-13, others you have to be at least 16.
However, the shops are allowed by personal preference and business practice to not modify any minor (even if the state allows)

Most states will require at the least some sort of signed paperwork stating consent and waiving liability. Individual tattoo parlors will most likely have their own rules complying with and/or in addition to whatever the state laws are. Some parlors will require the parent or guardian to be present at the time of the work being done.
You'd have to look up your state's specific laws, but standard procedure would be a consent form signed by at least one parent or guardian and signed waiver of liability.

As a general rule, states require signed parental consent. Even in the absence of a parental consent law, you will find that smart tattoo/piercing businesses will require it anyway.
You have to remember that there are potential medical consequences of either a tattoo or piercing. A minor would generally be deemed to lack the capacity to waive claims (agree not to sue) for a tattoo or piercing that goes wrong. A legal adult could sign such a waiver.

Generally, they need permission from their parents or legal guardians, who have to sign a waiver. However, they will not do any piercings or tattoos on or around the genital area for any minor, even with permission.

well it's different in different states. But i kno that in TN u have 2 b 18 to get a tattoo && i think u can b any age 2 get a piercing as long as ur parents r there 2 say u can.I kno cuz i have my nose done && i'm bout 2 get my tongue done && i'm only 13. But in KY u only have 2 b 16 to get a tattoo.

I asked my artist yesterday while he was tattooing my 15yr old son, he said here in Ohio there is no age limit with parental consent for a tattoo, his personal rule is no one under 16, he only did my son cause I have known him for 17yrs and he trusts my judgement when I say that my son is more mature at 15 than most 20 yr olds, and my son got a cross to signify his faith in Christ, so it wasnt like he was getting Eminem or something like that.
