I'm pretty sure he would like to know if you were sorry though. Just recognize in your mind that you have sinned every time you do in a small, silent prayer.

That is a common and very interesting question. We all sin. But the trick is to try to learn from your mistakes and try not to fall into the same trap again. There is such a thing as a man who "Repeats" his cycles. meaning he continues to do the same mistake over and over, never once realizing his mistakes, only by misfortune. Even I screw up and don't deserve God as a friend. But atleast he knows that down in my heart, I am striving to learn and do the best I can under any circumstances. So we still cool, man.

Not everytime. As Christians we are forgiven of sin wether we acknowledge it specifically or even if it completely slips our mind while we're praying. I pray at night and in the morning and ask for forgiveness of my sins, if possible I name them, but only because it shames me into trying to avoid it in the future. God already knows what you did, even if you can't remember it.

We all sin, but when we sin, we need to ask God to forgive us with sincerity, if we don't, then we aren't, it just an occasional, I am sorry, my bad, doesn't have any meaning. But to answer your question, Yes! But, don't take the free gift of forgiveness of sins for granted.
Galatians 5:13 - You, my brothers, were called to be free. But do not use your freedom to indulge the sinful nature; rather, serve one another in love.

I believe you have to ask forgiveness every time you sin, but i know that for me.. sometimes I don't even realize I've sinned until i go back through my week and take a moral inventory of my actions.. so my guess would be ask for forgiveness when your realize your mistakes.

No, don't ask for forgivness all the time. Then He will think you can't do anything, you just ask for forgiveness. He an't forgive you all the time. Do something to wipe off the sin. If you did a big one then ask for forgiveness then try hard to wipe off the sin.
