with the small diameter of the shaft, how do they make it hollow??

On the TV show How It's Made they showed them being rolled. I'm sure it would depend on the different manufacturer on how they decided best to make them.

Hypodermic needles are usually extruded, however there are some needles that are rolled. There is not a specific manufacturing procedure for hypodermic needles, most needles are extruded because is a much cleaner, quick and precise procedure.
