"our thoughts are with you" is the closest I've found to being appropriate. Think I'm going to buy a blank card and write some thoughts, but I'm not sure what I should say, any ideas?

Definetly a "thinking about you" card. Include things like what you have been up to, how school/ work is going. Also, try to subtly bring up fave memories that you guys shared (like this morning I did ___, it reminded me of when we ___)

A blank card is the way to go; make sure it has a picture of something nice reminding you of the person. On the inside you can explain why you chose it and share a favorite story about that person, what you admire or will remember most. Be sure to offer your support and how you would be willing to help out if need be.

you can take a photo of you and the person or one of just the person and make your own card, doesnt have to have words, just maybe a nice border. You should write something by hand such as, We will always be together, I'm thinking of you, our friendship is forever....or simply I love you. I think a store bought card is too impersonal...this could be your last goodbye with this person.

A "Thinking of you" card is always appropriate. You don't have to talk about their illness, but it would be nice to include a cherished memory you have involving the person.

Instead of sending a card, you should probably see them before their time comes, I'm sure they would value a visit more than paper.

Hallmark has a new card that says, "Thinking of you always, please remember me in your will." Seems appropriate.
