I visit Port Jervis every month which is just down the road from Middletown NY. I fly to Newburgh, NY (airport code SWF). Its 28 miles away from Middletown, its pretty cheap normally because AirTran and jetblue go there so all the other airline drop there price. Thats your best bet. I fly from Atlanta and its normally $190-$210 from there with airtran.
- The next closest is Westchester County airport (airpot code HPN) 68 miles, AirTran flies here too!
And the LaGuardia being the third closest 71 miles away (LGA airport code) I use LGA when I fly Southwest, very cheap airline.
SWF - 28 miles
HPN - 68 miles
LGA - 71 miles

Stewart Airport in Newburgh (code: SWF) is the closest one to Middletown. The airlines that service it are American, US Air, Jet Blue (only between there and Florida), and Air Tran. Delta starts flying out of there again in May.
After that, your next closest options are, in distance order: White Plains (HPN), Albany (ALB), LaGuardia (LGA), Newark (EWR) and Kennedy (JFK).
Stewart can be expensive to fly into or out of right now, until the Port Authority takeover is complete. But, if you fly into any of the other mentioned airports, you have to consider transportation to Middletown - so, that might cancel the flight difference. Average limo cost between this area and the major NYC airports (LGA, EWR, & JFK) is anywhere from $200-$400, renting a car will be about $100 or so.

The closest airport is in Newburgh NY - Stewart SWF - 20 miles
Its a pretty small airport and not sure what the fares are like
the next closest would be White Plains HPN about 55 - and Newark being the next at 75 miles
