Stop being crazy , look in the movies guide, and ask her if she wants to see a movie with you. If a meal, lunch, dinner etc, is part of the date, try not getting anything to eat that would be to awkward. (like spaghetti) > Take a deep breath< cleanse your brain and say it clearly and in a controlled voice, "Do you wanna see Dream Girls? It's playing at the Metro at 1 this afternoon. Can I pick you up?" You can do it! Just you and her. No friends for back up!

approach in a calm way and just start a casual conversation....ask her what she likes and what she thinks about things that has been going on. She will probably come around and tell her how she feels first if you do the right things.
