it would be the nationality of the parents. after birth, they would need to keep the woman and the baby warm, and give the baby a sponge bath, but i am sure they do train for this unlikely scenario! women are told by their doctors not to fly after the 8th month

As soon as the plane lands it will be met by an ambulance and she and the infant will be taken to a hospital. The baby will be given the same nationality as the mother.

The baby is the same nationality as the parents.
But, if it is a Tuesday at 3:54pm and you are flying over Italy, the baby is Italian!

If the plane is aloft, I believe the nationality of the child is whatever the nationality of the parents are.
