You should be able to find blackboard paint at home improvement stores -- like Home Depot and also at craft stores. I've seen the smaller bottles at Michael's.
As for surfaces; any smooth, flat surface will work fine. A piece of smooth plywood or fiberboard would probably work great. Be sure to read the directions on the container. You may need to prime your surface first.

both big home improvement stores carry it. you can buy magnetic base paint to go under it, or any other color, paint. i just did an entire wall with blackboard paint. It turned out really well

I find the best people to go through are these people. Any hard smooth surface should work out fine. One of my friends made his child's closet doors into a blackboard, which was really cool.

You should be able to purchase blackboard or chalkboard paint at any major home improvement store like Lowe's or Home Depot. Even stores like Ace Hardware, Wal-Mart, True Value and Menards carry chalkboard paint. You can purchase it in an aerosol form and also in quart size paint cans.
You should be able to paint any flat, smooth surface with it. FYI, you can also get magnetic chalkboard paint.

This paint is getting popular but I haven't seen it at the Big Box stores yet. Try the major paint stores such as Benjamin Moore and Sherwin Williams. It is more expensive than regular wall paint but serves it's purpose well.
