Sometimes a comforter is plain on both top and bottom, with the intent that they be put inside a decorative cover. The filling can range from polyester to feathers to down to a combination. Usually the feather or down comforters are plain.
Some people also call them bedspreads (although that's usually reserved for a single layer bed cover).

They call the duvets the same but very few ppl use them. Most americans have comforters which are simlar to patchwork quilts i.e they are padded and you can't change the cover on them.
