As to your second question, some countries have built large desalination plants to extract fresh water from sea water. These plants are expensive to build and run. It takes a lot of energy to purify sea water. For the moment these nations have plenty of energy in the form of petroleum. When the oil runs out they may be in serious trouble.

Some nations which are otherwise deserts do have localized fresh water sources such as wells or rivers. For instance everyone in Egypt lives within a few miles of the Nile. The rest of the country is empty. In other places deep wells have been drilled into aquifers. If you look at satellite photos of the desert you will sometimes see the tell-tail circles of pivot irrigation systems in the middle of nowhere.
