Generally, what cleavage means is that the bosom is ample enough, the bra lifts enough, or the shirt is low enough to show the area of the chest below where the breasts begin. It also generally means that the breasts are situated close together.
If you're looking for individual meaning--"What does it mean when she shows her cleavage?" it can vary. Some people show it to be sexy. Some people can't help it, because they have large chests and any v-neck or scoop-neck shirt will show cleavage. Sometimes it's just the fault of the combination of breasts + bra + neckline.
