I'm not sure but I believe the Dr. is deceased for he can't be found in the phone book.

Actually, most of these records are kept with the state and local health departments. Doctors are required to file these with them. You can find their phone numbers in the front of the phone book. Explain your situation to the local health department and they should be able to help you out.

Check with the medical board in your area first to see if he has moved. If you cannot locate him/her, did you have insurance??? If so, you can check your Explanation of Benefits sheet that they usually send after each dr visit, or call your insurance company for a medical claim history on your child.

I kept my own records of my daughter's shots along with copies of all of her medical records.

check with the ama maybe or the state medical board. he must have been liscensed through the state. they should be able to tell you something.
