It is a way of saying, what is that supposed to mean?

It can also be understood to mean, what is that supposed to be? Or, what is the meaning of this? It depends on the inflection you choose to give it.

Ironically you gave the answer yourself.

"¿Qué quiere decir esto?" translates as: "What does this mean?"


"What is this supposed to mean?" translates as: "¿Qué se supone que quiere decir esto?" So it's not the same thing as above.
