You will need all of the information you took when you got your passport the first time. You will want your drivers license, birth certificate and your old passport. Don't forget your checkbook or credit card!

If you're talking about a US passport, go to the State Department site to see what you need and download the application. If your passport was received within the past 15 years, you can renew it by mail. Here's the page for renewal in that case: http://travel.state.gov/passport/get/renew/renew_833.html
