Call the IRS at 1-800-829-1040, they can provide you with transcripts of your W2 information and have it mailed to you in 7 to 10 days, or by fax if you have access to a fax machine.

File an amended tax form, I cant remember what it is called. If you do not have the W2, ask the company for the W2. If they never sent to you and you havent gotten it after all this time, the IRS can send a letter threatening them with fees as penalties if they do not send to you. Go to www.irs.gov for more on this or call your Local IRS office to get more advice on this. You should only file the amended file if you had filed your 2005 taxes. However if you never filed it, file the taxes with the current w-2 and 1099 forms if you have them. I would contact IRS for assistance if you are missing a W2 that was never sent. If you need to re-request the form, please do that. Put everything in writing. Good luck!

Contact the employer who sent the W2. Their payroll department should still have those records.

You just about have to go to the original source (where you worked to earn the income).
You can go to or write to the Social Security Administration, but it will take quite a lot of communication!
