One of my patients was about 350 lbs when she started dialysis. Now she's down to about 220 lbs. It took discipline with her diet and A LOT of walking.
I have another patient who has been doing dialysis for about 4 years, and has lost about 75 lbs. Her thing is swimming. Now, she didn't swim very far or for very long at first, but little by little she gained endurance and the lbs came off. Now she swims 3 times a week and about 1 mile each time.
The important thing is to do something that you can tolerate, even enjoy, for a long, long time. I have another patient that yo-yo's a lot. He weighs about 370 lbs. He will gain 10-15 lbs, and then start eating salads for a couple weeks. Then he'll lose 25 lbs, and start eating McDonalds for every meal. Then he'll gain 30 lbs, and not know why. Avoid yo-yoing by being consistant.

I couldn't just sit around and do nothing like my doctors suggested.
They didn't want me to do anything or to take herbs or herbal remedies, but I had to try something - they just wanted me to do dialysis!
This program allowed me to take control of my health. I went from Stage 4 to Stage 3 kidney disease.
It was easy to do and my BUN, creatinine and anemia are all in better ranges.

"Weight loss" medication is crap anyway.
So, eat right. Sounds like that is the only thing you can do. Eat 6 very small meals, instead of eating 3 larger ones. Eat protein, vegetables, whole grains, fruits. In that order.

Drink water, lots of it. Your body needs it more than ever. For anything past that talk to your doctor as you have a serious condition and should not seek advice here. Good luck.
