Provide your cat with alternative places to climb & jump - buy or build some cat perches and trees. http://www.iroquoisinnovations.com/

Your cat needs to have places to climb, so providing it with places other than the counters and fridge will help. Then, you'll need to make the counters and fridge less appealing places for the cat to climb. There are several different ways to do that:

1) Cut some stiff cardboard to fit the surfaces you don't want your cat climbing on. Put some double-sided sticky tape on the cardboard, and then set the pieces of cardboard sticky-side up on the counters and on top of the fridge. The cat won't like the feel of the sticky tape, and as long as he has other places to climb (the cat trees), he'll learn to stay off the counters. You can remove the cardboard when you need to use the counter.

2) Put some shallow pans of water on the counters and on top of the fridge. When the cat jumps up there it will hit the water, which will discourage it from jumping up there.

3) Try a mat designed to repel cats - rubber mats with plastic bumps sticking up that are uncomfortable for the cat to walk on will discourage the cat from walking on those surfaces. Here's an example: http://www.drsfostersmith.com/Product/Prod_Display.cfm?pcatid=10301&N=2002+2017

4) Try an electronic motion detector device designed to frighten the cats away from places where you don't want them to go - these links have options:
http://www.drsfostersmith.com/Product/Prod_Display.cfm?pcatid=3453&N=2002+2017

Once the cat learns to prefer its trees to the counters, you can gradually reduce the time you have to put the deterrents on the counters & fridge.

My 2 cats are home alone all day, and the spray bottle only works when I am there. Since I'm working or sleeping for 2/3 of the day, it is effectively their apartment!

One thing that my holistic veterinarian told me is that cats hate citrus. I have some all natural orange oil air freshening spray that I bought at Walmart for less than $3. I spray it on my countertops after wiping them in the morning. You have to remember to wipe it up when you get home, though, because it's oily. It will work on wood tables or surfaces (I spray my door in the morning when they wake me hours before breakfast), but only for an hour or so, because the wood absorbs the oil.

After a couple of days of orange oil on the countertops, they should stay away for a while. I will do it again I catch one of them up there after that, but they usually "remember" for a few months. I've found this to be easier than putting duct tape on the counters, although I've done that too, with the wood tables in particular, and it helps. Good luck!

lots of good ideas. bottom line is just make it as uncomfortable for your cat as possible while hes on the counter. animals are quite simple. they want to be comfortable. so if your encourage good behavior with praise and make unwanted behavior as uncomfortabel as possible ( tape and water ideas both very good, loud noised or a sudden bang or a small slap on the bum work as well) they will come to associate the counter with negative things and will avoid them. But the key is to BE CONSISTANT. If you get lazy and allow your cat to walk on the counter even once. he'll know that your not really gunna back up your threats and then he'll just walk all over you.
And most of all it will take time with that consitancy. it wont happen over night so don't give up or the cat has won :)
good luck

Get yourself a spray water bottle. When she is someplace you don't want her give her a squirt with the stream setting not the mist. Pretty soon she'll get the idea. And she'll start to jump down when she sees the move toward the bottle.

Good luck

I use a spray bottle with water. Cats hate water. When I see them doing something I don't want them to do I just have to pick up the bottle now and they know it isn't a good thing they are doing.

Spray him with water, so he will know you don't want him to jump up the counter.

Many say a spray bottle, it depends on the cat. it has absolutely no effect on mine, he just looks at it like what was that? and goes about his business.
