I make yogurt smoothies, but the principle would be the same.
If you want a really cold milkshake or smoothie, you'll need to have cold ingredients to begin with, and preferably frozen. The blending action will generate heat which will make anything you blend get warmer and runnier.

So you can use ice cubes with regular fruit, or you can do what I like to do and use frozen fruit (either freeze your own, sliced/cubed/etc.... or just buy bags of frozen fruits already cut up). Then use only other cold ingredients, depending on how thick or concentrated you want it look or taste.
(You could also use fruit sherbet, or maybe even frozen real-fruit popscicles, etc.

Most milk will separate a bit if frozen or you could try that too, but ice cream could take its place.
