The most direct translation would be 前の恋人 (mae no koibito) but most Japanese would never actually say that. Usually 元か の (motokano, ex-girlfriend) or 元彼 (motokare, ex-boyfriend) is what you'll hear.

moto kanojo for ex girlfriend and moto kare or moto kareshi for ex-bf,some uses mae no kareshi(ex bf)and mae no kanojo(ex gf).You can even say MAE TSUKI ATTETA KANOJO-the girl i had an affair before or MAE TSUKI ATTETA KARE,the opposite meaning
