You don't pay for a medical card. We all have one from the day you were born (uk). It has a number on it which is just yours.
Entitles you to medical treatment in a nhs hospital

If its a small yellow NHS registration card then it entitles you to nothing.......everyone has one of these when they register with a doctor.
