You could get hepatitis, not to mention other diseases. You should get a hepatitis A and B shot at the very least.

Yes, you can catch and spread diseases through this method. Anything that was on your hands can now penetrate him and make him sick. Remeber, the rectum has fairly fragile skin (compared to the finger) so the risk of making micro-tears is very high. This increases the liklihood that he will catch something. Remember, fecal material exits through here, too, so you could catch e-coli, salmonella, and about a dozen other nasty diseases.
It is less likely that you will spread HIV through this method, but if your finger was in an HIV infected vagina, rectum, or blood right before it was in this boys anus, then you may have just killed him.
