The person who did my taxes in January still has not mailed me the state form so that I could sign and submit it. Can I go and have someone else perform my state taxes. My federal was done correctly.

Call them up and find out what is going on. Was your federal return a paper return or e-filed? If it was e-filed, maybe your state is going to be e-filed as well, or already has been filed.
Don't get your state done again if you have already paid for a state return with the first preparer.

Schedule an appointment to go into their office and sign it. Let them know you are unhappy with their service. Ask them to pay your expenses for going into the office. This should encourage them to finally mail you the form.
You want to avoid having two different versions of your state tax return submitted.

Well, you can, but you'd be paying twice for the same service. Call the person who did your taxes and ask them what the heck is going on.
