You can start by feeding her. Contact your local rescue organization about catching her. It is best to try and catch her before she has a litter. Otherwise, you will have even more to save.
They may be able to help you. They know the area vets and such that may help. Any kind of donation that you can give them would help greatly.

Well I found my kitten abandoned too when he was little. he was so small I had too bottle feed him. I wasn't planning on keeping him because of the cost and stuff. But I found out the humane society has a program that you only pay 15 dollars for a vet visit. It included him getting vaccinated, and examined, and later fixed when he was old enough. The program in my area is called Save the Felines, sponsored by the humane society and local vets. So I would check on your local humane societies web-site for a program like that. They should be very helpful for you.
In the mean time I would go get her and keep her in a crate or box with food and warmth for the night. Keep her in a garage or in a separate room from your other cat. She is probably hungry and needs your help. then in the morning call the humane society for some advice.

I wouldn't think it would cost a huge amount to just have the vet check her over and make sure that she's not sick. You are right not to want your present cat to catch something from her. It would be a good idea to have her spayed, also. If you can at least afford to have a vet check her out would be a good place to start. If you can't take her for yourself, then take her to a no-kill shelter so that she can find a good home. No kitty deserves to be abandoned. Bless you for caring about her! =)

I would get the cat and take her straight to the vet for a general check up and shots before you take her home ...some vets will offer a discount for stray animals..They should run a couple of tests to check for diseases and if she has anything really bad they might even recommend that she be put to sleep , if she does have anything it would be kinder to do this then let her suffer under an abandoned house...

Give her some food and gain her trust. She deserves someone to be nice to her. If she does not appear to be sick then she probably is not. Take her to the vet and get her checked out, a check up does not cost much. Maybe you could adopt her or take her to your local shelter.
You obviously care about animals as you say this breaks your heart. Please do something for this animal in need - even if it does mean taking her to the local shelter. It is better than leaving her there. The world needs people like you to take care of those who can not take care of themselves.

First off, call the humane society. Let them capture her and look her over. Then adopt her from there. That way you know she will have a clean bill of health and you wont have to deal w/ any "surprises". You are a good person for doing this. There is a special place in heaven for people like you.

If you can't take her in, try contacting a cat rescue. For instance, there is one in Baton Rouge, LA called Cat Haven. Maybe they can assist you in fostering the cat until another home can be found or you are able to adopt her, yourself.

Take her to the local animal shelter. They will check her out and medicate her if necessary.
Find out how long they keep animals before putting them to sleep and go back a day before the deadline. If she is still there, offer to adopt her, but tell them you can't afford to pay.
They will give her to you at that point.

if you can get some food it would be best to leave it near her but not to go too close. whwn she trusts you she may come into your home. you will have to get her to trust you because otherwise she wont go anywhere near you. she will probably be a bit jumpy around you but just be patient because a cat has been very jumpy as soon as he saw me but now if he will let me pick him up and he trusts me a lot hope this works for u
