i don't understand what you mean by not having patients.
like when they are not doing surgery they could be:
reviewing patients records, tests or x-rays and planning the surgery.
reviewing medical literature for the latest advances in the field.
observing other surgeons. writing research papers.
interviewing nurses to hire. buying a computer network for the office. completing paperwork for insurance companies.
filling out forms needed to practice at a certain hospital.
reviewing cases to see how surgery turned out.
reading professional publications.
researching which surgical tools to buy or use.
there are so many things to do besides the actual surgery.

A surgeon always has patients, so this question is completely hypothetical.

If they didn't have even one thing in the world to do, I suppose they'd go on to the house.

Play golf.
Surgeons don't generally work for the hospital at which they operate. They just show up for surgery. Patients are seen for consults in the doctor's office.

Most surgeons I know don't have all that much free time during the week.

They sub in for veterinarians and work on chimps, buffalo and skunks just to stay in shape No, that would probably rarely happen.
