I leave it open. I use a glass measuring cup or ceramic coffee mug in the microwave oven all the time and I've never covered them. I would think that might cause steam to build up and possibly scald you when you removed the lid.

The nature of microwaves is to heat from the center out, so placing a lid on it doesnt really matter, but if you go with a lid, watch for the steam.

If you cover the container, the steam that is emitted could cause the container to burst.
If you are measuring a specific amount, allow for a bit of the water to escape as steam.

I wouldnt totally seal it (the steam needs to escape or the lid will blow off)....you don't HAVE to cover it, I wouldnt bother with water, I only cover things (with "escape holes") if they are gonna end up splattering the microwave (like baked beans for example)

It's fine to cover it loosely (I use wax paper for this); if you cover it tightly, you can end up either blowing the top off because of the pressure, or getting steam burns when you open the container yourself - not fun.
