If you cannot file by the due date of your return, then you can request an extension of time to file. However, an extension of time to file is not an extension of time to pay. You will owe interest on any past–due tax and you may be subject to a late–payment penalty if payment is not made timely. To receive an automatic 6–month extension of time to file your return, you can file Form 4868 (PDF) by the due date of your return. For more information, refer to the Form 4868 Instructions.

You will never receive a W-4. You will however receive a W-2. You should contact your employer and request a duplicate to be sent to your current address.
If you need to file and extension - File Form 4868. That will give you until August 15th to file. And dont forget about your state extension as well.
Remember, an extension is an extension to file, not to pay. If you think you are going to owe some money on your tax return(s), Federal and State, make sure you pay them
by April 15th or actually April 17th this year - to avoid penalties and interest later.

you should have received it by now ,employee has up to January 31,2007 to get all w4 out, if you don't owe your are OK but if you owe the IRS please call for an extension
