Request a formal review if there isn't one already scheduled. Most places do these every 3, 6, 9 or 12 months, with the opportunity for a raise every 12 months.
Gather together proof of the work you've been doing and why you deserve a raise. Have you taken the duties of someone else who quit? Have you directly helped the company earn money? Top sales scores? Great customer feedback? Gather all this in paper form.
Once your boss agrees that you've worked hard and gone beyond what's required of you, propose a raise. Ask for more than you truly expect - if you want a 5% raise, ask for 10%, etc. so you have some room to negotiate.
Also, be willing to accept more paid vacation or paid sick leave, better benefits, 401k or stock options as well. For example, agree to a 5% raise plus an extra 3 days of paid vacation.
If the case is that you've been at a job for a long time and not recieved a raise at all, you can disregard the above and mention that your time and dedication to the company deserves at least a raise in tune with inflation. You might even find out what other companies pay people in similar positions.

ask him at a good time. walk in look him in the eyes and let him know you would like a raise and why you feel you deserve one if he says he cant because they only do a 3 percent yearly increase thats bs becasue he can change your job title around which would then entitle you to more money.

Do the quiet way. Work as hard as you can, and you might want to slip him a note- psst! I need to talk to you. Make sure you do all that. A good boss would notice your hard-working attitude and respect you for that.

I didnt like my boss.he was a Yankee.so one day I went in and acted like the boss,took the blueprints from his hand and told people what to do ,when to do it .etc.after a while production went up, he was fired and i was made boss.this brought me a raise in pay.try that.or just simply tell him you need a raise, after you do as best a job as you can do.show him what kind of worker you or.Good Luck.

you could start by looking at other companies and what they are paying someone in your job with your experience and credentials. if they are paying more then find a way to show your boss that you contribute to the company and that you deserve more. if he doesnt feel that you need more then go to the other company. if the other company is paying less then forget it.
