A 47 year old nurse presents to your office complaining of a poorly healing ulcer of her left second digit. The ulcer started a week ago and is painless. The patient has tried using over-the-counter antibacterial and hydrocortisone creams without improvement. She denies trauma to the hand. The patient has a temperature of 38.4ºC (101.1ºF) and left-sided epitrochlear and and axillary adenopathy. She has a 4 cm ulcer on the dorsal side of the left second digit covered by an extensive amount of non-pitting edema. Which of the following is the most likely diagnosis?

If it is a spider bite it would depend which part of the Country you live in."If you live in the United States"I live in Losangeles and I most people I've seen with a spider bite say they were bitten by a brown recluse.The only problem is Brown recluse spiders aren't found in California.It sounds as if it's a spider bite but without knowing the location of the Country it's very hard to say that it's from a brown recluse.

Knock A,C and D off the list.Now we're down to two possible problems.It's possible that none of there answers are correct.It sounds like a spider bite but I have to agree with little girl.It depends on where you live.If E. only said spider bite it could be correct.I feel your best answer is from little girl.

I'm a nurse. My guess would be septic (infection into her bloodstream) I guess you already know this could turn fatal. Did you run any blood tests and if so what did you find? Also did you do a culture on the wound for a proper diagnoses? Did she get any form of antibiotics? I hope this gives you some ideas on what to do for her. She could also be seen by a dermatologist refer her out. Take care.
