Rehab means rehabilitation and is a place where people go for many different reasons. Most people go to get help drinking or drug abuse problems, while others go for physical or mental therapy after having an accident or surgery or something like that.

So "in rehab" would mean he is in such a place to overcome his drink/drug problem. "Out of rehab" he has finished and has left.
