Needs assessment: What types of issues might indicate a need for training? From what sources would these issues be identified? If you were a training manager, how would you prioritize training ?

Are you a training manager? Good Lord how much experience do you have to have to ask these questions? I've been a nurse since 1981 (and am now a physician assistant) but is strikes me strange for the average nurse to ask these questions, and even stranger for a training manager to ask them. You need more experience as a nurse. And if you are not a nurse....talk to a few who are not wet behind the ears....they will have an abundance of answers. These are obvious questions.(and ...no offense intended but.....God I hope you are not a training manager)

Did the nurse complete a skills verification check list?
Assuming that the nurse is in a setting where she is responsible for one on one nursing care, you might monitor the care/procedures that she is doing, or check her documentation.
Has there been complaints from the clients or other staff members that can be validated?
Feel free to e-mail me is I can be of further help.

Is this a homework assignment? I hope there would be a list of classes each nurse has to take each year in order to stay current, and if the nurse is missing something from the list, they have to go get it filled in. otherwise you have to wait til they hurt someone before you know.
