here is some items that comes to my mind:
-having an infection in your teeth: check with your dentist.
-having residues between your teeth: you cant remove them just with brushing your teeth, you must use dental floss and antibacterial mounth washing too.
-having a problem in your digestive system: you must chech, I think if you dont have a dental problem it has the highest possibilities, maybe your pancreas doesnt work well or something is wrong with your liver.

Are you flossing too? this is very important, food gets caught in your teeth and decays. Floss every day!
if the floss doesn't help go to the dentist. They can check for any additional issues, and is the best place to start.
Also if you smoke- STOP! This does not help your breath either.

try brushing your tongue...that is where all of the bacteria is and that is what causes bad breath a lot of the time
so if you don't do it already, you should brush your tongue every time you brush your teeth...I am surprised your dentist hasn't told you this.

Well, consult a dentist. It may be cause of your diet (most possibly). And do you consume enough water everday? Also, always bring breathmints with you everywhere.
