Do you mean in or on? On top of the skin, soap works by forming little "bubbles" called micelles. These micelles trap dirt and bacteria. Soaps are also detergents and they can eliminate the charge of particles, which often kills cells. That is probably what would happen if you ate soap. But on top of the skin, you have protective oils, so the soap doesn't get to damage the skin. But you should avoid washing your hands too much. When I was sick I washed my hands a lot and they were chaffed for about a week afterward.

Soap probably works on the body because it is abrasive.
