You go to the department and ask for it. It's public information, and they should give you a copy, but there may be a small fee.

Yes. The police report will contain most, if not all, of the relevant information necessary to prove the elements of the crime.

Go to the department that filed the report and ask them for it.It might take a week or two.It costs around 5 dollars of coarse. Also when you recieve it ,look it over and make sure EVERYTHING is correct. Alot of police theorize things, therefore some facts might be left out .Especially if they are corrupt. But being as you have DUI much of it is based on evidence and you should be dealt with accordingly. Maybe after 2 duis' you shouldnt drink anymore. Or drive. Because you put other people in harms way for your negligent acts.

Here in Illinois, and in most other states, you can just go to the local police station and request a copy. You might have to pay some nominal fee, but they'll give you a copy of your report. It is after all a public record.

Police reports are public record. You can get it by paying the appropriate fees from the records section of the police dept.
