What does "twice removed' mean when talking about family?

when someone says, "she's a cousin twiced removed"?

It means 2 generations away.

A first cousin twice removed would mean that person is your grandfathers first cousin (but he is your 1st cousin twice removed)

it means that she has been in that family once then removed, in the family a second time then removed and now she is there a third time!
