When we say "I bet" we are borrowing an expression from the gambling world. When you bet, you are placing your money in confidence that your expectations will be realised, that your horse (or whatever it is you are betting on) will win.
So "I bet" means: "I am confident". "I bet that hurt" means "I am sure that that must have caused you pain." It is generally an expression of sympathy.

It means what it literally says, I bet that hurt. Unless your talking about English English because I'm American..

if someone was to drop a brick upon their toe, if one was to wager a dollar on the fact that the brick was to cause pain, then i'll consider the fact that the dollar was well invested.

It's an expression of sympathy. "I can understand that caused you/him/her/them pain"
