That's sounds really scary. I don't know, maybe he is seeing a ghost. I know some Christians think it is a demon spirit. maybe a demon spirit is following him. I know about schizophrenia, it could be that, i don't know much about it with kids. i would take him to a church first, a good calm church and have someone put a good spirit angel around him to guard him, than if that didn't work i would take him to a Chinese doctor maybe its his chi. last resort a doctor or counselor. you dont want to try meds unless u absolute must.

I wrote this poem for my son when he said that he saw monsters.
and it worked, maybe it will work for you :)
MONSTERS
What Are Monsters
Can You Tell
A Fairytale Character
Coming Out Of It’s Shell
The Shell I’m Saying
Has A Middle And Two Ends
They Can Be Hard Or Soft
It Really Depends
Please Don’t Be Frightened
I Know That You’re Scared
But The Monsters Can’t Hurt
Because They Know You’re Prepared
You Know What I’m Saying
Just Take A Look
When It Comes To Bed Time
You Just Close That Book!!!

That's great that he has such a wonderful imagination. Too bad that his imaginary "friend" is so scary.
Try to give your grandson back his power when he's dealing with the monster. Tell him that saying "Monster go away!" (or some other simple phrase) will make the monster go away (like when Dora says "Swiper no swiping!" and swiper goes away). Just make sure that you sound like know what you're talking about when you tell him and he will believe you and since it's his imagination, what he believes is what works.

Read him good story part of the bible at night and wait until he fall asleep. This gives him word insight from interesting stories. Other alternative soft music like ocean sounds, sounds of nature and some children music. Try to avoid exciting music that would wake him up. Children bible version is the best to start with, simple and clear! Music of nature and reading together really helps.

Seeing monsters is very common in children, he is a creative, intelligent child. This is his way of dealing with some stress in his life. Don't worry, just make him feel secure, and let him have some control in other areas of his life, like what to have for breakfast, or what to wear.

Make him some monster spray (water and an essential oil like lavender and put in some sparkles too) then give it to him to spray the monster to kill it. Spray the house so the monster can't come in. It works with many kids and "monsters" (ghosts too!)

It sounds to me like he is being harrassed by a demon. This happens. Are you a Christian? If so I would command it to go in the name of Jesus. No unclean thing can stand against the blood of Jesus. It would be good to take him to a church if you are not a Christian.

if you have taught him that God will protect him in all situations, that reinforce that. Teach him to pray for protection and pray with him. show him from the Bible examples of people who were protected from harm. don't let him watch scary movies, play violent activities and watch out that he doesn't associate with others who do such things. don't let anyone belittle him. talk to your minister about this.

Doctor or Priest, your choice. Inside line though is a good Pentecostal Church congregation will fix the problem in about 5 mins.
