I have lost 6 birth certificates in my short 19 years of life. I know first hand- to replace a lost birth certificate, go to your local health department, found in the phone book, and you have to bring your picture id, original social security card (no copies), and depending on your state $8- $30. (my state charges $28 for a new original one, $8 for photocopies of it).

If you know where (which state) you were born, call the records office in the state
Capitol for the details such as the address, what information is required from you
in letter form to obtain the birth certificate, and how much money to send with your request. If you are not sure where you were born, get a notebook and start calling
relatives. Chances are that some will remember correctly and some won't.

Call up or go in person to the "Register of Deeds" in the county that you were born in. They can mail you an ORIGINAL copy, not your hospital copy, for $10 - $20. By the way, if you plan on doing any traveling (foreign countries) or getting married you will need your original birth certificate.

Go to your health department with a social security card and driver's license and they'll replace it. It should cost about $10-20.

You can re-apply for them at the hospital you were born at, or you can re-apply for them (with ten bucks) at the office of records in your state's capitol
