Dark chocolate usually turns white when it has changed temperature like putting it in the fridge. It also turns white when it has melted a bit and then reset. Its not off or stale. But it does taste a little bit different.

Old? Probably. What happens when chocolate goes stale is that the crystalline structure of the once tempered chocolate is out of whack. If this is cooking chocolate or a solid bar, you can re-temper it, thus re-aligning the crystals.

Yes. Some of the fat has worked its way to the surface. This does not mean that it is spoiled; chocolate keeps very well.

actually it's totally fine to eat chocolate that looks like that. it's just oxygen interacting with the chocolate. it's not as fresh as it could be, but it's not bad for you and will still cook and taste the same.

Yes. I'm not sure why, but it is. Some people will tell you it's gone bad, and others will tell you just to melt it down and call it good. I'm in the 2nd camp.

Its either old, or it has gone bed because of fluctuation in temperature.
