Just attach your old passport with the valid visa under the new passport.I hope the old passport no. is also mentioned in the new passport?There should be a stamp on your new passport with the old passport no. mentioned.....If this stamp is there then you have nothing to worry about?Just fly to the US.....

No, you do not need to inform the US embassy. When you travel to the US, though, you will need to present both passports - the old, canceled passport, which contains the valid US visa, plus your new, valid passport, which does not contain a visa. NOTE: If the US visa in your old passport is damaged or defaced, it will not be considered valid, and will need to be replaced.

it is a good thing that indeed you have a new passport , for now ,to visit the USA ,you need the new passport ,(law)
as far as you worrying about you contacting the US ? no , you do not have too ........when you new passport was done ? the researches have been done on you ......
besides , if you did not have the new passport done ? you entry to the USA would have been refused ......if you are worry about this ? contact the USCIS........

Your passport is not old. It is your present passport. Apply for duplicate by paying 2500/-
If Visa is damaged, you need new from embassy.

You have to apply for a new US visa since the old one was already damaged together with your old passport.

It is not necessary. In the new passport it will be mentioned, that passport holder has travelled / holding so and so passport (your old passport No. will be mentioned in this page). Officials will accept this. Don't need to worry.
