First is kissing, second is him feeling your breasts, third is him feeling your vagina, home is intercourse.
This is very old school. It goes back to when guys were trying to be discreet while talking in the open, so they used baseball terms to talk in code about how far they went. They would use other phrases as well to describe things they did, like a double play, a foul, etc.

3rd base = sex acts other than penetration (oral sex)
home run = full intercourse
