I met this girl randomly in the mall, what should I do?

So it was around 4 months ago that my sis and I went to a mall, at A&F store, I met this very extremely cute girl. She gave me the kind of impression that I have never had before, she's perfect for me. I was too chicken to ask her out, or even a number. We didn't have a conversation, but she smiled at me when we made our eye contact. When I finally got the courage to ask her, my sis wanted to leave, then she promised me that she will come back with me to store again, but when we did, she left. I regreted it so.. soo much. Then, the following two weeks, I came back to the mall trying to see her, but she was never there. I assumed that she quit the job. Recently, my sis told me she saw her there again, so I am going to take my chance on next Tuesday. How should I start a conversation with her, how do I ask her number, or ask her out, I knew nothing about her, I don't even know if she has a boyfriend, should I ask her at the counter with customers and co-workers, if not, what should I do?
