Unemployment compensation that you receive will be reported to you on Form 1099-G. List the amount from Box 1 on the appropriate line titled "Unemployment Compensation" on your tax return -- Line 19 on Form 1040.

If Federal income taxes were withheld, they'll be listed in Box 4 of the Form 1099-G. Add that amount to any W2 or other 1099 withholdings on the appropriate line -- Line 64 on Form 1040. If tax was withheld, you must enclose a copy of the Form 1099-G with your tax return if you file a paper return. If you can't e-file with a PIN and have to mail in the transmittal form to the IRS, enclose a copy of the Form 1099-G with your W-2 forms and the transmittal form.

You should have received a 1099 from the state. That goes in as income on the 1040.

You should have rec'd a 1099-g.

This amount should be entered on the line for: Unemp Comp

If you use: Form 1040EZ - Line 3
If you use: Form 1040A - Line 13
If you use: Form 1040 - Line 19

It goes on your 1040 or 1040A. There's no additional form to file.
