If you have Windows XP then you already have a good speech dictation/recognition system built in. Additional capabilities can be added using the e-Speaking (website below) Voice and Speech Recognition software.
Microsoft has a good summary on their website called: "How to install and configure speech recognition in Windows XP" (see last reference below).
The combination of Microsoft's software and e-Speaking should enable you to accomplish great things with voice recognition.

Look up Dragon's Naturally Speaking or IBM's ViaVoice. If you have MS Office (think it started with Office XP), you should have a language toolbar that would let you input into MS Office apps already.
Oh, these only apply to MS Win32 based systems. Not sure if either application supports Mac or Linux.

Yes.
The two most known are ViaVoice from IBM and Dragon Naturally Speaking.
It used to be that you could get the core engine of the viavoice system free, but it's pretty impossible to find the engine that was released that way anymore.
