In the US you need to go to your local post office. You can find all of the information that you need at http://www.usps.com/ As far as getting your pricture taken, there a tons of places that do that most CVS, Walgreens, Walmarts, etc....

In America, go to your county clerks office. There you will fill out a form. You will need to bring your birth certificate, and a 2 inch photo (you can have it taken at many post offices or photo shops, even most AAA stores) Since you are 16, you will need to bring one of your parents with you. I believe that it will take you 60 days to get a passport; however, you can get a rush one for twice the price. The price of a standard passport, when i purchased it, was 67 dollars. I also believe that you have to pay by money order. If you call your local county clerk, they will tell you exactly what you need to bring with you. you can get your passport photo taken in the post office and boots etc and train stations. Remember to get the picture checked at the post office to ensure that it will be ok with the passport office before you get it signed etc.
The post office offers a passport check service which is always worth it.

In the UK, apply to the passport office. Get a form from your local post office. Get your pictures taken there as well, or at a big railway station with those photo-me booths, or some Boots, shopping centres, etc.

As soon as you get to this site it gives you a link to click that says what to do as an American citizen to get your passport.
As far as passport picture you should be able to get it done at any photo shop or even drugstores like CVS and Walgreens.
