I'm a bit worried by this question- i hope it doesn't mean you have just found out a terrible secret in your family. I'm afraid the other answers are correct though, somebody has some explaining to do.

Yes, but it's slightly more unlikely.
Each of the parents may have O+ on one chromosome, and a different blood type on the other. The child can inherit either. This means the child could inherit a different blood type quite easily.
And of course, there's genetic mutation to take into consideration. I don't know how much difference there is genetically between O+ and B+ in terms of genes, but 'mistakes' can be made during development.

So the answer is yes

Impossible, genetically.

O is reccesive, meaning that a person needs to receive an O type gene from both of the parents.

If the parents are BOTH O blood type, they can only transmit the O gene.

One of the three tests is mistaken I guess.

Yes it can, as genetics plays an important role on this. If one of the parent or both have relatives or ancestors that have kind of blood type its possible.

No it is not possible. "O" blood group has no antibody of type A or B hence, parents of "O" group will have babies of only "O" group.
