The most common reason for going outside the litter box is a urinary tract infection (UTI). The cat feels pain when peeing and associates this pain with the box, so she tries to go elsewhere to see if it will hurt less. So the first step is to take her to the vet to check for a UTI.

Also, is he/she spayed/neutered? If not, get that done asap. Unfixed cats will pee to mark their territory.

If both of these reasons have been ruled out, the next step is too take a good look at the litterbox itself. Is it cleaned? Scooped at least once a day, with the litter exchanged completely whenever necessary?

Is it in an easily accessible spot, where the cat feels safe? It should not be in a high-traffic area of your home, or next to anything that might scare the cat, e.g. the washer.

Further, many cats do not like to urinate and defecate in the same spot. Adding a second litter box would solve that problem. (Actually, the general rule is that one should always have one more liter boxes than cats - so, for one cat, two boxes.)

Is it possible that she doesn't like the kind of litter you use? Many cats dislike crystal litter. If the litter is scented, get an unscented one. cats noses are much better than ours, and often, what seems like a nice pleasant scent to us can be on overwhelming stench to the cat.

Is you cat declawed? Many declawed cats avoid the litter box because the litter hurts their paws. If that is the case, try using a really soft litter, such as "Yesterday's News", which is made from old newspaper.

Otherwise, you could also try using Cat Attract litter - it has special substances added to it that make sit smell to the cat like an attractive place to "go".

Have you moved the litter box, changed litter, or had some change in your house?
All of those things can cause a cat to stop using the litter box.

If none of those things have happened take kitty to the Vet, it's possible she is suffering from a Urinary Tract Infection.

Also you need to make sure that you get the smell out of the carpet completely...a product called "Natures Miracle" works wonders for that.

Good luck.

Take it to the vet and tell the vet what has been gappening. Then ask what he/she thinks the problem is and how to treat it. He/she might give you a prescription for you pet to see if that helps and let the vet know the results. Then go from there.
