I have looked everywhere and it seems as though Da Vinci doesn't have an epitaph on his grave. He was buried in the Church of Saint Valentine at Amboise, France. He was wildly popular in his time but I don't believe it actually says anything on his grave.

Allegedly his last words were "I have offended God and
mankind because my work didn't reach the quality it should have."
But there is no epitaph on his tomb just his name and a metal plate with his picture.Leonardo da Vinci's Tomb is in Saint-Hubert Chapel (Amboise) France.
