I actually just had to do this recently. When I requested a free credit report I noticed an account on there that I did not open even though it was closed out I wanted it removed from my credit report. I filled out an online form to dispute saying I had never opened it and a week after they took it off and sent me a letter saying they did so. You can also probably call any of the 3 credit bureaus (Experian, Equifax, or TransUnion) to get that straightened out.

file a dispute with the credit reporting agencies. experian, equifax, and trans union. sorry, don't have the #'s handy but they are online. the credit reporting agencies will contact the creditor for proof of the late pays, repos, etc. and if the creditors can't come up with proof, they will be corrected on your credit bureau w/in 60 days or so. hopefully this has taught you to constantly monitor your credit report, as it affects so many things nowadays.

It is important that you submit a letter to the credit bureau (s), Certified Mail-Return Receipt Requested, disputing the validity of the debt in question.
State your reason for the dispute and request that they remove it from your credit record.
In addition, request in your letter that they, (the credit bureaus), re-send you an updated copy of your credit record showing the correction.
