Aspirin is the standard because it relieves not only pain but also inflammation from arthritis. However, it can cause bleeding of the stomach lining, especially in high doses needed for arthritis. Ibuprofen is similar in action to aspirin but causes less bleeding and lasts longer than aspirin. Finally, acetaminophen, like Tylenol, is the easiest on the stomach but can cause liver damage in high doses and does nothing for the inflammation. So my pick would be the ibuprofen for an over the counter drug to help relieve arthritis pain and inflammation.
