Where can I find the W-2 Form for the Telephone Excise Tax?
I am told I can file for a refund, how do I do that, if I have already filed my 2006 return?

If you already filed the IRS will pick up on the fact that you did not claim the Credit and will make the adjustment for you. This should have been claimed on the return you filed. Due to you not claiming it, your return will be delayed while they make the adjustment. Depending on the number of exemptions on your return You should see an increase in the amount of your refund

If you are filing a return, then you claim it on whatever version of 1040 form you are filing - there's a line for it in the "PAYMENTS" section of 1040, 1040A and 1040EZ. If you are NOT filing a return, you can file a 1040EZ-T just to claim the telephone excise tax refund. You are NOT allowed to file both.

If you already filed a 2006 return without claiming it, then the only way to still get it is to file a 1040X amended return. Since not everyone is eligible for it, the IRS will NOT just give it to you automatically.

The credit is $30 for a one-person household, up to $60 for a four- or more person household. If you would have to have a paid preparer do your amended return, think whether their fee would be higher than what you'd get, in which case you'd be ahead to just forget it.

You have to file an amended return (1040X). You could have claimed it as a line under tax credits on your 1040. W-2 has nothing to do with this.

The IRS will not give it to you unless you claim it. They have no way of knowing whether you paid for long distance service unless you tell them.
